What does a Support Coordinator do?
We pride ourselves on offering a personalised service to our clients. Some of these services may
include:
Explaining your NDIS plan to you in easy to understand language
Helping you to set your NDIS portal
Helping you to navigate the NDIS price guide
Explaining your Service Agreements, including the one you have with us
Sourcing providers and therapists if required
Helping you have a more successful and less stressful plan review
What do we need from you?
To provide you with the best service we request that you do the following:
If not completed already, please request your LAC to send us a Request for Service to Nelle Frances to enable us to be your Support Coordinators.
Prior to receiving services from the Nelle Frances Team, there needs to be a service agreement completed . We will then complete a Service booking for you. After a Service Agreement is in place with the Nelle Frances team you are able to access your NDIS MyPlace Portal, select ‘Share Plan Consent’ by going through my Service Providers and selecting Nelle Frances, or by calling the NDIS and requesting them to share your plan with us if you give permission to do.
If applicable, send a letter of termination to your previous Support Coordinator.
